To express your interest in volunteering, please submit the following to the Human Resources Department:
- Volunteer Application
- Community Service Workers Application
- A recent Tuberculosis clearance (results must be within 6 months)
- Copy of official photo I.D. (e.g. identification card, driver’s license, passport)
- Dates of Availability (days and times available each week; start and end dates)
- Two personal references
